Just For Fun Tournament Waiver
All players participating in the Just for Fun Tournament must sign our liability waiver prior to playing in their first game.
We have moved to an electronic waiver system. Please use the link below to sign the waiver for your player. The electronic waiver system will be available until 3pm Friday September 4th. After that time, hardcopy waivers must be submitted by coaches at Tournament Registration. Our Registration tables will have the waiver status for each team available for comparison to your roster. Hard copy waivers will be available at Registration for players that still need to complete them.

Waiver status updated 9-4-26 3PM – Final Update. CSC will periodically update the waiver status link leading up to the tournament. The final status update will be posted on the afternoon of Friday September 4th. At that time the hard copy waiver will be available for download.

This form is currently closed for submissions. Please complete a hard copy waiver (located at Registration and on the Website) and provide it to your player's coach prior to their first game. All players must have a signed waiver before participating in the tournament.
